Architectural Review Request

Please use this form to request a change to your lot that requires approval of the HOA Board. Remember to drop off any needed drawings, documentation, etc. to the ARC VP in order to complete your request. We strive to process these requests in a timely manner, however, the Board has 60 days to respond to your request as documented in the Covenants. Please plan accordingly.

Project	Please select the project(s) you wish to complete by checking the appropriate box(es). Please answer all questions related to a specific project to ensure prompt response from the ARC.

Install Satellite Dish (Section 6.c.xvi)	Check this box if the following 3 statements are TRUE. * This dish I wish to install is 18" or less in diameter. * The color of the dish will be dark gray or black. * The dish will not be visible from the street.
	If you have checked the above box, then you are approved to install your satellite dish and may proceed with installation. Please complete the "Acknowledgements" and "Signature" sections at the bottom of this form and hit submit. Failure to install the dish as indicated above will result in penalties up to and including a lien on your property until the violation is corrected. If you have not checked the above box, please describe the details of your request below in the "Details of Request" section of this form.

Tree Removal (Section 6.c.xvii)	No approval is required for any tree that is within 10 feet of a drainage area, a septic field, a sidewalk, a residence, a driveway or the line formed by the highest normal pool elevation of any lake. No approval is required for trees less than 4 inches in width. NOTE: If the tree is a flowering tree (ie Dogwood), you must receive Board approval even if less than 4 inches in diameter.

Fence Installation (Section 6.c.xxiii)	Please be sure to read the Fence Guidelines as documented in the Supplemental Guidelines document before proceeding with this request.
	The style of fence that I am requesting matches the style of the nearest house with a fence per the Supplemental Guidelines.
	Please document the address of the nearest house with a fence to your property.
	The fence I am requesting does not adhere to the guidelines as documented in the Supplemental Guidelines. I have documented the reason for this out of guideline request below.
	I agree to install this fence enclosing our entire property. If you are requesting to install the fence and not enclose your entire yard, do NOT check this box and detail your request below in the "Details" section.
	Please note: After receiving approval to install your fence, uou must adhere to all of the above requirements, the requirements stated in the Supplemental Guidelines for Fence Installation, and all local and county requirements. Failure to install your fence as indicated above will result in penalties up to and including removal of your fence at owners cost, daily monetary fines, and/or a lien on your property until the violation is corrected.

Exterior paint change.	Please provide information on your new color selections below in the "Details" section. Include manufacturer, color name, and color number. Indicate location of each color. EXAMPLE: Front Door, Sherwin Williams "Black", EXT1234
